in Orchard Park is a residential nursing facility providing long-term and subacute care. The facility opened in 1994 and is dedicated to the memory of Monsignor Nelson Baker.

in Tonawanda (on the Kenmore Mercy Hospital Campus), is named after the foundress of the Sisters of Mercy, Catherine McAuley. The facility provides long-term and subacute care services.

in Buffalo (adjacent to Sisters of Charity Hospital) is named after St. Catherine Labouré, Daughter of Charity, who devoted her life to providing care for the elderly. The facility provides long-term and subacute care services.
